We seek a driven Licensed Practical Nurse to be part of our compassionate team s position requires a commitment to delivering goal-directed care in alignment with provincial standards. You'll be tasked with conducting clinical assessments, evaluating care plans, and effectively communicating with care teams to ensure optimal service delivery for all residents.

Key Responsibilities:

• Implement and adjust resident care/service plans

• Conduct thorough clinical care assessments

• Provide medication and treatment administration

• Assist with resident care conference scheduling

• Facilitate transitions for residents with the care director

Requirements:

• Practical Nurse Diploma and valid registration

• Over 3 years of experience in geriatric care

• Up-to-date CPR certification

• Skilled in Microsoft Office applications

• Demonstrated leadership and coaching skills
