Interior Health is searching for a reliable and motivated LPN to work at Vernon Jubilee Hospital. In this role, you'll collaborate with multidisciplinary teams to plan, implement, evaluate, and coordinate patient care transitions. Key Responsibilities:

• Lead care transition activities for assigned patients

• Assess patient needs with families and health teams

• Collaborate with diverse healthcare professionals

• Practice within BCCNM standards and guidelines

• Provide guidance on appropriate services and resources

Requirements:

• Graduation from an accredited Practical Nurse program

• Three years recent clinical experience in Community and Acute Care

• Current BCCNM registration and valid B.C. Drivers License

• Flexible availability for casual shifts

• Commitment to patient-centered care
