As a Transition Liaison Registered Nurse, you will handle complex patient care while fostering collaborative relationships with healthcare professionals. This position requires two years of nursing experience in acute and community settings, focusing on improving patient outcomes through effective communication and resource management.

Key Responsibilities:

• Deliver independent nursing care to patients

• Consult on patient eligibility and care service transitions

• Develop and coordinate patient access activities

• Establish communication between healthcare services and patients

• Identify and resolve case-related issues proactively

Requirements:

• RN degree from an accredited School of Nursing

• Minimum of two years recent clinical nursing experience

• Must be registered with BCCNM

• Hold a valid British Columbia Driver's License

• Readiness to work casual hours as needed
