ROLE PURPOSE

This position will provide coordinated and accessible support and services to recipients of CBCHB rent supplements in market housing. This role will assist with improving housing stability through an integrated service provision approach and aid vulnerable individuals with maintaining housing in the private rental market.

STATUS AND CLASSIFICATION
Unionized. This position is open to applicants of all genders.

LOCATION

Vernon, British Columbia
- Must be able to travel throughout the interior

SHIFT HOURS

Full Time (35 hours per week):
Monday
- Friday (8am- 4pm)

REPORTING AND WORKING RELATIONSHIPS
  • Reports and is accountable to the Property Manager or their designate
  • Liaises with staff and volunteers of community and government agencies as required
  • Liaises with community landlords and the private market housing sector
  • Liaises with the staff, managers, and on-call manager of Connective as necessary
KEY

ACCOUNTABILITIES
  • Participate regularly at the BC Housing led coordinated access and assessment table
  • Responsible for reviewing and either accepting or deferring referrals received through BC Housing and the coordinated access table process
  • Conducts referral consultations based on criteria, urgency and other factors to aid with a successful transfer or referral process
  • Data collection, input and analysis of case notes for use in financial reviews of assets and declarations of annual income
  • Provide coordinated and accessible support services in market housing, including liaising with landlords and reviewing eligibility and suitability of referral into potential landlords
  • Triage support efforts for those that are precariously housed and in emergency circumstances
  • Work with service users under a person-centered model that fosters opportunity to move from temporary housing or precarious housing situations and into market housing under an individual services model
  • Provide coordinated case management services, physical health, mental wellness and substance use supports, social, emotional and community supports, personal care and personal living supports, food security supports, indigenous cultural supports
  • Provide direct nonclinical supports to recipients and work with community partners including the health authorities
  • Ensure supports include all aspects of daily living, including but not limited to personal life, health, housing, leisure lifestyle and access, employment and education
  • Promote and be an advocate for independence and growth through a shared design of a flexible service model
  • When and as needed, provide a wide range of strategies to enable a service user to address and accomplish the established goals of improving housing stability, good neighbor approaches, support with communication plans and guest conduct. Further personal goals (ie. Securing a job, reestablishing relationships with family members, developing skills with management of garbage , recycle collection and local bylaws)
  • Model effective interactions with service users, including positive rapport building and support outlined within the integrated support framework
