The Laurel at Vernon Hills Memory Care, located in Vernon Hills, IL, is seeking a dedicated and passionate CNA to provide high-quality care to the residents in our community.

CNA Certification Required (ALL SHIFTS NEEDED)

Responsibilities of the Care Partner

Provide assistance with activities of daily living in a manner that supports our person-centered care philosophy.

Complete resident rounds as assigned, reporting any resident care issues to the Nurse on duty.

Receive report from the previous shift and medication aide.

Read the shift-to-shift report and shower schedule for information and changes before coming on shift. Document in the shift-to-shift report book and ADL binder before going off shift.

Provide personal care services as outlined in the individual services plans such as assistance with bathing, grooming, dressing, ambulation, positioning, assist to meals, feeding, change bed linens, laundry, etc.

Assist in daily requirements and tasks as requested.

Ensure residents are provided privacy, respect, and dignity.

Follow standard precautions and infection control procedures.

Come to work as scheduled and consistently demonstrate dependability and punctuality.

Complete state-required training upon hire and annually.

Required Experience

Possess, at a minimum, a high school diploma or equivalent.

Must have IL State approved CNA Certification.

Must possess, or be able to obtain current CPR certification.

Be at least 18 years of age.

Experience working in Senior Living, Memory Care, Home Health, or Long-Term care environment.

Have the ability to spend long periods on your feet and to ambulate quickly.

Able to work independently and on your own from a day-to-day basis.

Complete all appropriate documentation accurately and timely.

Basic computer skills.

Must be able to speak, read, and write in the English language.
