Join us at Hartford Health Care as a Physical Therapist (Rehab, Therapy) – Homecare. This role involves providing high‑quality physical therapy to patients in their homes under written orders from qualified physicians, adhering to agency policies and regulations.

General Summary of Duties: Provision of physical therapy to patients in their homes in compliance with Agency policies and written orders from qualified physicians.
Typical Physical Demands: Requires full range of body motion, heavy work, balance skills, stair use, floor‑to‑stand transfers, kneeling/sitting for extended periods, lifting/carrying up to 50 lbs, correct vision/hearing, working under stressful conditions, and limited exposure to communicable diseases.

Conditions:
Work is performed in patients’ homes and in an office environment.
- Admits/readmits patients and provides physical therapy in compliance with Agency policies and State and Federal Regulations.
- Conducts a complete physical assessment.
- Develops and implements a care plan and therapeutic goals with the patient/family and arranges for assistive equipment.
- Evaluates patient safety on a 24‑hour basis.
- Applies appropriate clinical judgment in coordination of patient care by all services involved.
- Continuously evaluates significant changes in the patient’s condition and progress toward therapeutic goals.
- Utilizes Agency and community consultants/resources.
- Instructs patient/family in procedures, responsibilities, and optimal wellness practices.
- Plans for the patient’s discharge from service.
- Maintains case mix weight, utilization, and patient satisfaction aligned with industry standards and Agency objectives for quality care.
- Ensures OASIS documentation correlates with the Plan of Care.
- Uses recommended OASIS assessment techniques 100% of the time.
- Maintains OASIS, documentation, and Plan of Care correlation at 95%.
- Documents all services in the clinical record within State and Federal regulations and Agency policy.
- Exports documentation and mileage, if appropriate, within 36 hours of visit.
- Obtains physician verbal/written orders and complies with all reporting requirements.
- Participates in coordination of care with physicians and other disciplines as appropriate.
- Manages work assignment efficiently and effectively.
- Contacts client and conducts first home visit upon receipt of referral.
- Maintains flexibility in daily planning to allow for emergencies or unexpected visits.
- Reports cases at risk to the Clinical Manager and responds appropriately.
- Holds required productivity expectations as established by the Agency.
- Attends interdisciplinary team meetings and informs primary nurse of the client’s progress/status.
- Delegates tasks to Home Care Aide and orients/supervises them per State, Federal, and Agency policies.
- Participates in interdisciplinary Plan of Care for hospice patients when appropriate.
- Utilizes payer sources correctly, reflecting changing levels of care.
- Delegates patient care responsibilities to the Physical Therapy Assistant when appropriate.
- Demonstrates understanding of case management principles and ability to manage cost and quality.
- Develops and maintains sufficient professional skills and credentials to meet the needs.
Of Patient Care
- Attends a minimum of 12 hours of in‑service per year, including mandated annual in‑services.
- Participates actively in self‑appraisal and the formation of a personal development plan.
- Adheres to the Agency Corporate Compliance Plan and HIPAA Privacy Plan.
- Seeks appropriate clinical input to ensure proper patient care.
- Projects a professional image through attitude and compliance with the dress code.
- Is capable of precepting new Therapists and students.
- Works effectively as a team member with consideration for all staff.
- Provides the Agency with a current copy of the professional license.
- Maintains CPR certification.
