Position SummaryResponsible for the accurate flow and processing of referrals for home health, palliative care, and hospice care. Responsible for the overall management of the intake department, including performance management and process improvement.
Qualifications- Graduate of an NLN-accredited school of nursing; BSN preferred.
- Possession of a current license to practice as a registered nurse in the State of Connecticut.
- Two (2) years’ full‑time clinical experience, at least one of which was in a home‑care agency.
- Knowledge of agency policies, principles and state/federal regulations.
- Competent in clinical health principles and ICD‑10 coding.
- Good interpersonal skills – able to work positively with staff at all levels and function independently within scope of responsibility.
- Capacity to implement agency goals and directions and to maintain relationships with referral sources.
- Oversee the intake department processes:
- Ensure referrals are accepted on a timely basis.
- Document referrals and RN starts on the shared board.
- Maintain highest quality and professionalism to all customers.
- Supervise day‑to‑day function of intake nurses.
- Identify and obtain all necessary information for referrals:
- Assess W10 and all documentation from referral sources.
- Obtain verbal orders for SOC with detailed patient condition and services.
- Assign accurate ICD‑10 codes.
- Collect history, physical and all required documentation for coding.
- Coordinate all disciplines/services for successful initial assessment.
- Obtain necessary documentation for face‑to‑face visits.
- Maintain positive relationships with referral sources within hospitals, physician offices, and healthcare facilities:
- Serve as information resource for home health services.
- Assist complex patient conferencing as needed.
- Provide early intervention resources and foster cooperative relationships.
- Act as liaison between referral sources and payors.
- Perform other duties as assigned.
- Oversee and manage the performance of referral nurses in the department.
