Acts is currently seeking qualified candidates for our Home Health Occupational Therapist position.
- In-house program within a welcoming, slower-paced community environment that delivers holistic care with a strong focus on prevention
- Specialized on‑site training delivered during paid work hours, with all sessions eligible for CEUs
- State‑of‑the‑art rehab gyms and equipment
- Opportunity to build relationships by following residents across the full continuum of care, including inpatient, home health, outpatient, and fitness services
- Clinically driven, patient‑centered treatment decisions guide all programs and care plans
- Cross training provided to support all rehabilitation areas within our communities
- Limited travel within and between our private communities

In this role, you will be responsible for providing occupational therapy services to home health residents/clients in their home environment to promote functional independence, safety, and quality of life. You will evaluate residents/clients, develop individualized plans of care, implement evidence‑based interventions, and collaborate with the interdisciplinary care team to achieve optimal outcomes in compliance with physician orders, state practice acts, and federal and state home health regulations.
RequirementsThe ideal candidate will meet the following requirements:
- Master's degree or Doctoral degree in Occupational Therapy from an accredited program
- Minimum of 1‑2 years of experience in home health is preferred
- Valid Occupational Therapist license or eligibility for license in the state(s) where services are rendered

$74,160.00 - $93,463.00 / year. Starting rate will vary based on skills and experience.
