If you're a licensed Physical Therapist (PT) passionate about helping patients live healthier, more independent lives at home, we'd love to connect with you!

As a Physical Therapist, you'll work 1:1 in patients' homes, helping them regain strength, mobility, and confidence. You'll be a key part of a collaborative, interdisciplinary team focused on whole-person care.

• Develop and implement individualized treatment plans based on physician orders
• Deliver patient-centered physical therapy in the home setting
• Educate patients and caregivers to support recovery and long-term wellness
• Document visits clearly and promptly in our EMR system
• Participate in interdisciplinary care conferences
• Maintain compliance with agency policies and healthcare regulations

• Active Florida PT license (in good standing)
• Current CPR certification
• Level 2 background screening
• Negative TB test or chest X-ray
• Solid knowledge of physical therapy best practices
• Strong communication and collaboration skills
• 1+ year of experience (multidisciplinary setting preferred)
• Experience with Alaya Care EMR is a plus
