• Provide skilled physical therapy services, including pelvic floor therapy, in patients' homes under the supervision of a Physical Therapist
• Carry out individualized treatment plans for pelvic floor and functional mobility conditions
• Assist patients with therapeutic exercises, mobility, balance, and functional activities
• Educate patients and caregivers on pelvic floor exercises, home programs, and safe mobility techniques
• Document patient progress, outcomes, and responses to treatment
• Collaborate with the interdisciplinary care team to ensure coordinated, high‑quality care

• Active Florida Physical Therapist Assistant (PTA) license
• Pelvic Floor certification (required)
• Degree from an accredited PTA program
• Valid driver's license & reliable transportation
• Current CPR certification
• Strong communication, documentation, and time‑management skills
• Home health experience (preferred)
