You will work independently within the Primary Care Network, applying your advanced counseling skills and clinical knowledge to assess and manage patient care effectively.
Your role includes addressing barriers, promoting health education, and collaborating with various healthcare professionals to optimize patient wellbeing.
Key Responsibilities:
• Evaluate patient needs and barriers to care
• Develop and execute comprehensive care strategies
• Provide crisis intervention and grief counseling
• Foster partnerships with healthcare teams and community services
• Educate patients and families on available resources
Requirements:
• Bachelor's Degree in Social Work required
• Minimum of two years of experience with complex clients
• Valid BC Driver's License is necessary
• Familiarity with chronic health challenges and community principles
• Ability to adapt and communicate effectively with diverse stakeholders
