Position: Licensed Practical Nurse Role

Key Responsibilities:

• Conduct patient histories and nursing assessments

• Monitor patients with acute and chronic health conditions

• Perform clinical procedures including wound care and injections

• Administer vaccinations and prescribed medications

• Advocate for patient safety and independence at home

Requirements:

• Licensed Practical Nurse with current BCCNM standing

• Clinical nursing experience, preferably in geriatrics or primary care

• Strong communication and interpersonal skills

• Valid driver's license and reliable vehicle

• Commitment to relationship-based care
