Reporting to the Coordinator, this LPN role involves planning and evaluating holistic health care for Indigenous clients. You will engage in developing client care plans, offering health counseling, and advocating for client and family needs. Building connections with various service providers is key to ensuring culturally competent care tailored to community and hospital contexts.
Key Responsibilities:
• Develop culturally safe client care plans and conduct assessments
• Facilitate access to health services and community resources
• Provide health education, counseling, and wellness promotion
• Collaborate with healthcare teams to enhance client care
• Maintain accurate documentation and records
Requirements:
• Current LPN registration with BCCNM
• One year of recent nursing experience
• Knowledge of Indigenous health protocols and cultural issues
• Valid BC Driver's license and vehicle access
• Strong assessment and communication skills
Leverage your nursing skills to support Indigenous health initiatives and improve community health outcomes.
