Client Case Worker - Rock Bay Landing

The Client Case Worker acts as an access point for residents into the broader spectrum of housing, employment, substance abuse treatment, medical, and community mental health systems in Victoria and elsewhere. The Client Case Worker works in consultation with the program manager to plan,develop, and implement client programming and activities. This role is expected to provide the program manager with ongoing client case management updates, andto maintain accurate and complete client files and notes.
To succeed, you will have an excellent understanding of theissues faced by our client group. This includes experience working with individuals from diverse backgrounds who face multiple barriers, such as substance use, mental health concerns, low incomes, and negative impacts of colonialism and racism.
Qualifications
- Degreeor diploma in a related human or social services field.
- Two(2) years of direct service experience working in a program with:
- Client-centered case management.
- Creating and implementing case plans.
- Collecting and reporting program data and maintaining client files.
- Program development and facilitation.
- Linking individuals to various housing and treatment programs.
- Demonstratedability to develop effective relationships following harm reduction principles.
Requirements
- Successful completion of the Ministry of Public Safety and Solicitor General Criminal Record Check.
- Class5 Driver's License with a clear driver's abstract.

Hourly Wage Rate:$32.02 - $35.52.
Shift Time: 7:00 am to 5:45 pm.

Portability
