Community Support Worker

Building Bridges Community Support offers support and services to children, youth and adults with diverse abilities in Sooke and the Greater Victoria area. Our services include offering practical assistance with skills development, advocacy, and connection to community and its resources. Building Bridges Community Support operates from of a foundation of inclusive, strength based, person-centered and trauma informed practices. A Community Support Worker meets with individuals one on one in their homes and the community. The role involves developing and implementing person-centred support plans and assisting the individials in working towards their goals. The Community Support Worker maintains accurate records, completes reports, and participates in team meetings and community outreach activities. This position also requires advocating for individuals’ needs, promoting community inclusion, and responding to urgent support needs when they arise.

The position includes flexible working hours to meet participant needs. Hours are dependent on the individuals and their contracts.

• Minimum of 2 years experience working with diverse populations, including individuals living with physical and developmental needs, and/or facing mental health challenges, substance use, homelessness, or social isolation.
• Strong interpersonal skills and the ability to build trusting, professional relationships with diverse populations and community partners.
• Excellent communication abilities, including clear verbal communication, active listening, and concise, professional documentation.
• Ability to work independently and collaboratively within a multidisciplinary team, with strong organizational and time-management skills.
• Valid driver’s license and reliable vehicle.
• First aid certificate.
• Competent computer skills and access to a private computer.

• Trauma-informed, and aware of culturally safe practices.
• Case management skills, including assessment, goal planning, coordination of services, and ongoing monitoring of participant progress.
• Understanding of community health, including social determinants of health, local resources, and community-based services. Experience with marginalized or vulnerable populations is an asset.
• Diploma or degree in Social Work, Community Support, Human Services, Psychology, or a related field.
