Key Responsibilities:

• Provide daily emotional and physical support to clients

• Develop and implement case plans for individuals

• Associate with clients during emergency situations like overdoses

• Advocate and connect clients to community resources

• Facilitate referrals and maintain client records

Requirements:

• Diploma in Social or Human Services field

• Minimum two years' relevant experience in community services

• Emergency First Aid with CPR Level C certification

• Clear Criminal Record Check required

• Knowledge of community support agencies
