Essential Functions & Responsibilities
  • Serve as medical director for skilled nursing facilities (SNFs) and participate in administrative and quality initiatives for the facilities they oversee
  • Attend and participate in monthly quality assurance and process‑improvement meetings; supervise facility clinical staff and review/revise clinical guidelines; ensure compliance with state and federal regulations; train facility staff
  • Act as attending physician for residents at each facility
  • Collaborate with NPs and PAs to manage day‑to‑day resident care
  • Provide high‑quality, patient‑centered, comprehensive primary care services by rounding on patients residing in SNFs
  • Diagnose and treat patients with acute illness, such as bacterial and viral infections, and chronic diseases such as hypertension, heart disease, or diabetes
  • Complete provider documentation in Theoria’s proprietary EHR system (Chart Easy™) and other commonly used facility EHR systems (Point Click Care , Matrix Care, American Health Tech)
  • Participate in Theoria’s chronic care management, transitional care, RPM and other medical quality initiatives and programs
  • Supervise collaborating NPs and PAs
  • Utilize electronic/online medical tools such as EHR systems and state/county medical systems for death certificates and administrative functions

Shift Structure
  • Shifts are flexible depending on physician schedule; typically 2‑3 days per week at each facility
  • Rounding times are flexible with no set start and end time; physicians prefer to start between 7‑10 a.m. and finish when caseload is seen
  • Call is covered by Theoria's virtual care team
  • This is an in‑person position
Qualifications

MD or DO, licensed to practice in applicable state. Ability to perform essential functions with or without reasonable accommodation. Other qualifications as required by the employer.
