From Pain to Wellness, LLC is an interdisciplinary chronic pain management center dedicated to providing comprehensive care in chronic pain management. Our mission is to enhance the quality of life for our patients through effective pain rehabilitation, medication management, and functional medicine strategies.

Role Summary

We are seeking a Nurse Practitioner or Physician Assistant to join our clinical team at From Pain to Wellness, LLC. In this role, you will be responsible for evaluating and managing patients with chronic pain conditions in collaboration with our physicians and interdisciplinary staff. You will play a central role in longitudinal patient care, contributing directly to treatment planning, medication management, and functional restoration.

This position is designed for an APP who wants ownership of patient care within their scope, values interdisciplinary collaboration, and practices with a functional, whole-person approach to chronic pain.

Responsibilities
  • Conduct comprehensive patient assessments and physical examinations for chronic pain conditions
  • Manage established chronic pain patients under physician supervision
  • Develop and implement individualized treatment plans aligned with clinic protocols and care philosophy
  • Prescribe medications and treatments in accordance with state regulations and clinical guidelines
  • Monitor patient progress and adjust treatment plans in collaboration with the care team
  • Educate patients on pain management strategies, expectations, and lifestyle considerations
  • Collaborate closely with physicians, therapists, and other clinical staff to ensure coordinated care
  • Facilitate referrals to other healthcare professionals and diagnostic services as appropriate
  • Maintain accurate, timely, and compliant clinical documentation
Qualifications
  • Licensed Nurse Practitioner or Physician Assistant in the state of Illinois, or eligible for licensure
  • Previous experience in chronic pain management or related specialty medical practice preferred
  • Experience with medication management for complex patient populations
  • Comfort managing a full patient panel within a structured clinical environment
  • Strong communication skills and ability to build rapport with patients
  • Ability to work collaboratively within an interdisciplinary care model
  • Commitment to following clinical protocols, documentation standards, and compliance requirements
  • Experience with or exposure to functional medicine principles aligned with chronic pain care
