Position SummaryThe Case Manager Lead will provide supervision to staff who provide skill building/case management services to assigned cases of Family Health Center patients and their families. The Case Manager Lead will assist with onboarding of new staff members and monitor continuous training for staff who provide skills training. Services will be provided in the community or client’s home.
This is a School Based Position*
Essential Duties and Responsibilities- Review and oversight of assessments of assigned youth and their families within the timeline identified by policy.
- Review and oversight of the development Person Centered Treatment Plans (PCTP) and treatment plan reviews with youth and their families within the timeline identified by policy.
- Review and oversight of services as outlined in the PCTP including case management and individual/group skills training.
- Mentor and train all MRO staff.
- Responsible for rounding to all designated work areas and to provide oversight and support to staff.
- Provide guidance on entering progress notes, complete State Registration and CANSA documents, maintain schedule in EPIC, and complete billing documentation within the timeline identified by policy.
- Will be responsible for covering extended leave and or provide coverage in special needs situations.

- Develop and maintain positive relationships as well as provide collaboration and advocacy for focused driven youth and family outcomes with both internal and external customers.
- Maintain all training and certification requirements.
- Effectively collaborate with community members and partners to ensure well rounded and quality patient care.
- Engage in additional community training as identified by the organizational needs
- Mental:
Attention to detail. Possess critical thinking and the ability to manage multiple projects lity to communicate clearly both verbally and in writing with the treatment team as well as outside agencies. Must be able to engage in cooperative relationships. - Physical: frequent and prolonged sitting, manual dexterity, and visual and hearing dexterity.
- Skills & Abilities:
Sensitive and flexible when dealing with adults, adolescents, and children with a wide range of affective and behavioral disturbances. Case Manager Lead must demonstrate knowledge of identification of needs, symptoms, and behaviors of individuals served - Working Conditions:
Clean well-lighted, heated, and ventilated office. Will also work in the client's home, other community agencies, and provide transportation in their own vehicles at times.
Education: Bachelor’s degree in a human services field.
Experience: 4 years of active case management experience working in a behavioral health setting.
