Skills Coaches provides skill-building and supportive services to clients and their families to promote independence, stability, and successful functioning in the home and community. This position assists clients in developing and strengthening skills related to daily living, health and safety, communication, social functioning, parenting, education, employment, and overall wellness. The Skills Coach works collaboratively with clinical staff, clients, families, and community partners to reinforce treatment goals and support positive client outcomes.

This position is a 1.0

FTE, working 40 hours a week.

essential functions

The essential functions may include, but are not limited to the following:

  • Supports and adheres to the Organization's Mission, Vision, and Values.
  • Provide skill building and supportive services to clients and families as identified in the plan of care, including individual and group-based services.
  • Assists clients in developing and strengthening independent living, health and safety, social, parenting, and daily functioning skills.
  • Engages clients and their social support in community-based activities and resources that promote independence and positive outcomes.
  • Collaborates with clients, families, clinical staff, and community partners to support identified treatment goals and client needs.
  • Participates in team meetings and communicates with internal and external team members regarding client needs, progress, and services.
  • Maintains positive and professional relationships with clients, families, coworkers, and community partners.
  • Helps clients navigate transportation, appointments, community resources, and other services identified as part of their plan of care.
  • Provides support during transitions, including discharge from hospitalization, transitions to independent housing, and changes in schools or educational settings.
  • Reinforces coping strategies, wellness plans, and other skills identified by clinical providers.
  • Follows designated curricula when conducting skills training and parenting sessions.
  • Maintains regular contact with clients and families by approved communication methods and within established work schedule expectations.
  • Monitors client progress and communicates relevant updates to the treatment team.
  • Enters accurate and timely documentation into the EHR and completes required billing documentation within established timelines.
  • Maintains knowledge of Medicaid Rehabilitation Option (MRO) guidelines related to service delivery, documentation requirements, qualifying diagnoses, and level of need.
  • Maintains required training and certifications and meets organizational expectations related to productivity, documentation, policies, and compliance.
  • Maintains appropriate professional boundaries and protects the confidentiality of client information.
  • Performs other duties as assigned.

  • Experience working with individuals and families in behavioral health, community-based services, or a related setting preferred.
  • Experience working with diverse populations, including individuals with complex medical or behavioral health needs preferred.
  • Experience working with individuals in recovery and/or families preferred.

  • Able to perform essential job functions effectively.
  • Frequent sitting, standing, walking, and reaching throughout the workday.
  • Ability to perform repetitive motions, including typing and computer use for EHR documentation.
  • Ability to travel to client homes, outreach offices, community agencies, and other community locations as required.
  • Ability to safely transport clients when required as part of assigned duties.
  • Ability to lift and carry items weighing up to 20 pounds as needed.

Working Conditions:

  • Clean, well-lit, ventilated office environment.
  • Work may also occur in client homes, community agencies, outreach locations, and other community settings.
  • May require transportation of clients by organizational vehicle or other approved means.
