Inpatient Case Management RN

Overview

Sentara Princess Anne Hospital is hiring an Inpatient Case Manager RN to work full‑time 40 hours per week. The role is responsible for the provision and facilitation of comprehensive care management, services and quality outcomes for patients across the healthcare continuum, with a primary focus on patient transition to the appropriate level of care while ensuring clinical appropriateness and length of stay.

The position directly supports in‑hospital patients identified as needing comprehensive case management, promotes effective utilization and monitoring of health services, collaborates with the healthcare team and patient/caregiver to manage care and transitions, and develops and implements a comprehensive care plan based on assessment and evaluation of physical, mental and psychosocial needs of the inpatient and caregiver.

• 12 months of nursing experience
• Registered Nurse
• Basic Life Support (within 90 days)
• BSN preferred
• Case Management certification (ACM, CCM, CCCTM, CMAC, or CMGT‑BC) required within one year
• 1 year acute care case management preferred
