We are seeking an experienced Nurse Practitioner or Physician Assistant to provide evidence-based primary care to patients with cancer in both office-based and virtual settings in Virginia Beach, VA. This position works closely with oncology providers and an interdisciplinary care team to coordinate comprehensive care, manage chronic conditions, and improve patient outcomes.

Key Responsibilities
  • Provide direct,evidence-based primary care to patients with cancer in office and virtual settings.
  • Collaborate closely with oncology providers managing each patient's cancer care.
  • Manage chronic conditions and complex medical needs, including congestive heart failure, frailty, and behavioral health.
  • Coordinate referrals andcare with primary care and specialty providers in the community.
  • Provide transitions-of-care visits and support continuity of care.
  • Participate in interdisciplinary team huddles, case reviews, and care coordination activities.
  • Escalate clinical concerns to the supervising Market Medical Director when appropriate.
  • Collaborate with clinical and practice leadership to improve clinical operations and patient care.
  • Support quality initiative sand achievement of key clinical quality measures.
  • Perform other related responsibilities as needed.
Requirements
  • Current certification andlicensure as a Nurse Practitioner or Physician Assistant.
  • 4+ years of direct patient care experience in primary care, oncology, or a related setting.
  • Strong experience in chronic disease management and/or palliative care.
  • Oncology orcomplex/high-acuity patient care experience preferred.
  • Excellent clinical,communication, care coordination, and teamwork skills.
  • Ability to work effectively in a growing Value-Based Care environment.
