Position Overview

Eastern Healthcare is seeking a skilled and dedicated Regional MDS Coordinator to oversee MDS assessments and the care planning process for our long-term and skilled nursing facilities located across the East Side of Virginia. The ideal candidate will have significant experience in MDS coordination within skilled nursing facilities (SNF) and be able to ensure accurate, compliant documentation while optimizing reimbursement opportunities under Medicare and other payer sources.

This position will require strong clinical knowledge, attention to detail, and the ability to collaborate with facility staff to ensure the highest standards of care and regulatory compliance.

Key Responsibilities
  • MDS Assessment Coordination: Ensure that accurate and timely MDS assessments are completed for all residents, in compliance with Medicare, Medicaid, and other regulatory requirements.
  • Care Plan Development: Collaborate with interdisciplinary teams to develop, implement, and review individualized care plans based on MDS assessments and the clinical needs of each resident.
  • Regulatory Compliance: Ensure compliance with all federal, state, and local regulations, including Medicare and Medicaid guidelines, to secure optimal reimbursement for services rendered.
  • Documentation Review: Audit MDS documentation for accuracy, completeness, and consistency with clinical findings, ensuring it supports care plan goals and reimbursement.
  • Quality Assurance: Monitor and evaluate the quality of MDS assessments and care plans, ensuring best practices are followed and corrective actions are taken when needed.
  • Staff Training and

    Education:

    Provide ongoing education and training to facility staff, including nursing and therapy teams, regarding MDS processes, documentation, and care plan development.
  • Collaboration with Leadership: Work closely with facility leadership and the regional clinical team to identify opportunities for improvement, implement new processes, and maintain high standards of care.
  • Travel: Travel to facilities within the East Side of Virginia as needed to conduct MDS assessments, audits, and provide training.
Qualifications
  • Registered Nurse (RN) with current licensure.
  • 2+ years of experience in MDS coordination within skilled nursing facilities (SNF).
  • Strong understanding of Medicare, Medicaid, and other payer requirements for MDS assessments and reimbursement.
  • Experience with care plan development and interdisciplinary team collaboration.
  • Familiarity with electronic health records (EHR) and MDS software platforms.
  • Excellent communication, organizational, and problem‑solving skills.
  • Ability to work independently and manage multiple tasks in a fast‑paced environment.
  • Availability to travel within the East Side of Virginia as required.
