Occupational Therapist – Home Health | Gate City, VA

. We’re currently seeking an experienced and compassionate Part‑Time Occupational Therapist (OT) to serve patients in Gate City, Duffield, Nickelsville, Scott, and Wise, VA
.

• Provide skilled occupational therapy services in accordance with the physician’s plan of care.
• Implement therapeutic interventions designed to restore daily function and independence.
• Instruct patients and caregivers on alternative strategies for daily activities, safe transfers, and proper positioning.
• Coordinate care with other healthcare professionals to ensure continuity and optimal outcomes.
• Accurately document patient progress and submit records within 24 hours of each visit.
• Maintain compliance with agency policies, procedures, and home health best practices.

• Licensed Occupational Therapist in the state of Virginia.
• Minimum of 1 year of clinical OT experience (home health experience strongly preferred).
• Strong clinical judgment and communication skills.
• Commitment to patient‑centered care and interdisciplinary collaboration.
• Reliable transportation, a valid driver’s license, and required auto liability insurance.
