Case Manager
Under general supervision of the Clinical Director, the Case Manager is responsible for providing service to a variety of customer groups, including physicians, allied health professionals, facility emergency departments, convenience care centers, other health‑care providers, schools, social service and mental‑health agencies, police, courts, employers and the public Case Manager will have knowledge of FAPT, CSA, and other community agencies as well as insurance carriers.
The DSM‑V is applied to ensure that appropriate clinical criteria are used for screening potential residents. Demonstrates skill in using clinical assessments and evaluations to determine an appropriate disposition for placement.
The Case Manager collaborates with the treatment team and community partners to address treatment progressions and overall completion of treatment planning. Coordination of care and discharge planning are central responsibilities. Through review of documentation, the Case Manager serves as a liaison and facilitator between the resident, physician, other treatment team members, and reimbursement agents to ensure that treatment resources, goals, progress, and related matters are effectively utilized and communicated to the resident and all parties involved, so that each resident’s length of stay is appropriate.
The Case Manager also provides case‑management duties as assigned, notifies families and community members of any changes in treatment team schedule, submits monthly updates to referral sources and families, and works within the clinical department to maintain the facility’s continuum of care. Responsibilities also include providing recommendations for discharge planning, partnering with outside providers for continuous care, facilitating family engagement, supporting facility initiatives, and offering actionable feedback and advocacy.
Additionally, the Case Manager partners with the Marketing Team to identify outreach opportunities and participates in community events, health fairs, and speaking engagements to promote the facility’s services.
Maintains strict confidentiality and privacy of residents’ personally identifiable health information in accordance with HIPAA privacy rules and facility policies, and meets mandatory staff development requirements per regulatory and accrediting agencies.

- Bachelor’s degree required.
- Strong written and oral communication skills required.
- Must be at least 19 years of age per Commonwealth of Virginia Interdepartmental Regulation of Children’s Residential Facilities.
- Minimum of one (1) year in a behavioral health / therapy setting with the population served.
- One (1) year’s experience in intake and patient assessments (preferred).
- One (1) year’s work experience in the treatment/treatment planning process (preferred).
- Cardiopulmonary resuscitation (CPR).
- First Aide certification.
- Behavioral Management program completion within 14 days of hire.
- Valid Driver’s License in good standing.
- Facility is well‑lit, air‑conditioned, and heated.
