Personal Care Aide

Delivers quality home health care services to the Agency’s clients under the direction of a Registered Nurse in accordance with physician orders, Agency and industry standards, and federal and state regulations.

• Assists members with activities of daily living, including bathing, dressing, grooming, toileting, eating, transferring, and mobility.
• Engages members in meaningful conversation, socialization, and activities while providing personal care assistance.
• Responds to all members' needs and/or requests for care promptly, and provides or obtains assistance with care as necessary.
• Observes members for changes in physical, emotional, mental, or behavioral condition, unusual symptoms, accidents, injuries, or unusual occurrences, and reports promptly to a licensed nurse.
• Completes all forms and documentation such as incident reports, change of condition reports, and behavior tracking, as requested, and in a timely manner.
• Prepares and serves meals, assists with eating, excluding tube feeding.
• Assists with self-administration of medications.
• Makes and changes bed linens.
• Cleans immediate living areas including bathroom and bedroom and removes trash.

Education and Experience:

• CPR Certificate (preferred)

• High school diploma or equivalent

• Previous experience working with the elderly in a residential or long-term care setting is also preferred.
