Care Coordination of a caseload of service users with complex mental health needs. Working with internal and external organisations to improve outcomes, manage risk and deliver effective care. Supporting service users to reach optimal levels of mental wellbeing, engage in local communities and achieve personal targets. Work with a small creative and dynamic team who offer excellent support to each other.
Deliver mental health interventions and treatments in the community. Requires a resilient, passionate and empathetic team player who can think outside the box to encourage engagement of service users that present with complex mental health needs.

Main duties of the jobContribute to excellent care for our service users. Develop and lead on care planning risk assessment and formulation. Deliver treatment interventions in the community. Work closely with our inpatient unit to support safe discharge. Initial assessment of service users being referred to the service. Supporting depot treatments both in community and within depot clinics. Working closely with partner organisations. Referring to and taking part in safeguarding processes.
Supporting less senior members of staff and students in supervisory roles. Keeping clear and accurate records. Supporting compliance with our clinical governance framework.

Job responsibilitiesJOB SUMMARY
Has continuing responsibility for the provision of skilled interventions to people with mental health problems in the community. Works within a multi‑disciplinary team as a member of an Enhanced CMHT.
Key Result Areas- Management of own caseload in line with C.P.A. procedures and risk management protocols.
- Provision of leadership to the Enhanced Team in respect of psycho‑social interventions to people and their carers in line with national and local practice standards e.g. NICE.
Supervision and support of other team members. Participation in service development and clinical governance structures.
ProfessionalAssesses needs and identifies problems relevant to the care of people referred. Devises a plan of care in partnership with the person and carers.
