Registered Nurse – Infusion Coordinator About the Role

Responsibilities- Ensure timely patient progression of infusion patients from hospital to home.
- Act as the first point of contact for new home health referrals and clinical coordination.
- Review clinical documentation and referrals to ensure appropriateness and readiness for home infusion services.
- Coordinate with physicians, hospital discharge planners and the interdisciplinary team to develop individualized plans of care.
- Serve as the front‑line resource for patient and nursing staff questions.
- Support quality outcomes and continuity of care through proactive communication and planning.

- Education:
Graduate of an accredited nursing program required. Bachelor of Science in Nursing required within 5 years of hire. - Experience:
1 year of relevant experience. - Licensure:
Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia required. Valid Virginia Driver’s License required. - Certification:
American Heart Association (AHA) Health Care Provider BLS certification required.
Job may require sitting for prolonged periods, frequently standing, traveling, bending/stooping. Proficient communicative, auditory and visual skills; attention to detail and ability to write legibly. Ability to lift/push/pull 20‑50 lbs. May be exposed to chemicals, blood/body fluids and infectious disease.
Position Compensation$82,992.00 - $ Annual
