Range Mental Health Center is proud to be a Certified Community Behavioral Health Clinic (CCBHC). The goals of the Certified Community Behavioral Health Clinics program in MN are to
1) increase access to community-based mental health and substance use disorder services (particularly to under-served communities),
2) advance integration of behavioral health with physical health care, and
3) improve utilization of evidence-based practices on a more consistent basis.

This role is direct patient care and administrative in nature, designed to meet the needs of our community consistent with our organization’s status as a CCBHC. The Licensed Practical Nurse will provide care across the organization, though it will most likely be focused on one area for most of their time. Programs that they will be involved with as part of this position include substance use disorder services (detoxification and inpatient residential treatment services) and outpatient psychiatry.
Duties/Responsibilities- Licensed practical nurses (LPNs) work closely with supervising nurses and providers to assess and monitor patients’ needs and ensure treatment and care are conducted according to the facility’s procedures by:
- Caring for patients
- Records a patient’s medical history, symptoms, and vitals with prompt and professional documentation.
- Updates patient charts and electronic health records (EHR).
- Performs medical tasks, including blood draws and administering shots, and responds to emergent medical needs such as first aid, CPR, wound care.
- Administers medication and other treatment team-recommended treatment plans.
- Complies with federal, state, and local nursing regulations, standards, and policies.
- Collaborates with fellow staff, other departments and outside agencies.
- Completion of continuing education requirements. Participates in yearly performance review process and provides peer reviews as directed.
- Adherence to organization code of conduct, maintaining appropriate communication and boundaries with other staff, patients, and outside agencies.
- Other duties as assigned are consistent within the LPN scope of practice, meeting the needs of the organization and patients.
- Provide chart audits, policy review, updating of policy and procedures as requested to assure compliance with regulatory requirements and assuring safe delivery of care. Report findings to fellow nurses, clinical nurse manager, program director, medical director and other stakeholders as appropriate.
- Provide reinforcement of learning to unlicensed assistive personnel on medication administration, documentation of medication and health assessment, CPR, withdrawal protocols, etc.
- Regularly attend program, treatment and staff meetings, providing recommendations for improvement in patient care, program services, organizational needs.
- Collaborate with all inter-organizational departments to facilitate prompt and safe delivery of services to patients.
