Our Hospice team has an immediate opening for a part time Admissions and Liaison Nurse to be part of our expansion to the northern Virginia area.
Responsibilities:- Perform initial assessment of patients' needs
- Obtain fully informed consents before admitting patients to the hospice program
- Serve as a clinical resource for referral sources to patients/family members
- Facilitate end of life conversations and decision‑making with the family or caregiver.
- Prepare and discuss with a physician an initial plan of care for patients
- Establish working relationships that promote utilization of the Hospice by hospitals, physicians, community‑based case managers and other community agencies.
- Represent our agency well when communicating with patients, families, physicians, and when participating in community network meetings.
- Flexible work schedule – must be available Monday through Friday to complete up to 5 admissions a week.
Qualifications:
- An active RN license to practice in the State of Virginia
- A minimum of 3 years of acute care and/or hospice experience
- Demonstrated knowledge and skills necessary to provide meaningful care to and communicate with the terminally ill
- Excellent communication, customer service, attention to detail, and strong organization skills
- Proficient with Microsoft Office 365 applications and the Homecare Homebase electronic medical record system
- A valid driver’s license, clean driving record, and reliable/independent transportation to provide care to hospice patients throughout northern Virginia
