Responsibilities

• Oversee collaborative requirements of service delivery for children birth to age 3 years with developmental delays, atypical development, or conditions that may result in a developmental delay.
• Coordinate and implement assessments for service planning, including the development, review, and evaluation of Individualized Family Service Plans (IFSP).
• Coordinate and monitor the delivery of services in partnership with medical and other providers involved with the child.
• Facilitate the development of transition plans when the child leaves early interventions services.
• Perform required on‑job travel.

• Minimum Bachelor’s degree in Human Services (e.g., social work, psychology, special education, or related fields) or education.
• Basic computer skills.
• Knowledge of and ability to identify community resources and activities that support service delivery and family participation, Part C regulations, and skills to coordinate and monitor services for infants and families.
• Experience working with infants and toddlers who may have developmental delays or related family service experience.
• Must complete all required Early Intervention (EI) training modules, including EI Targeted Case Management, within 6 weeks of hire.

• Knowledge of service delivery, including coaching, to infants and toddlers with developmental delays.
• Knowledge of child development.
• Ability to coordinate and facilitate multidisciplinary team activities.
• Skills in conflict resolution and effective written, oral, and interpersonal communication.
• Knowledge of client record documentation and Part C system requirements.
• Ability to build rapport with families, including communication, listening, and problem‑solving with families from diverse socio‑economic backgrounds.
• Ability to identify and coordinate community activities to support family participation and inclusion.
• Bilingual (English/Spanish) candidate preferred.

Position is a grade 208. Starting hourly wage is $24.60 per hour. Offers will be commensurate with knowledge, skills, and abilities subject to budgetary constraints.

Monday – Friday, 8:30 am – 4:30 pm. Telework:
