The Family Housing Case Manager I – Residential Programs advances HFF’s mission by providing participant-centered, housing-focused case management and supportive services to families and youth experiencing homelessness. The position works directly with participants throughout their involvement with HFF, helping them identify housing needs and barriers, develop individualized housing action plans, access community resources and financial assistance, and secure and maintain permanent housing.
This position ensures case management services are responsive to participant needs, focused on achieving strong housing outcomes, compliant, and aligned with HFF’s mission and program requirements.
The Family Housing Case Manager I – Residential Programs works collaboratively with participants, Housing Navigators, other case managers, landlords, Coordinated Entry, and community service providers to connect households with housing and supportive resources, address barriers as they arise, and provide ongoing support that promotes successful transitions from HFF’s residential programs to long-term housing stability.
Position SummaryThe Family Housing Case Manager I – Residential Programs provides direct housing-focused case management services for families and youth experiencing homelessness participating in HFF’s residential housing programs. Responsibilities include participant intake and assessment, development and implementation of individualized housing action plans, housing search and placement assistance, supportive services during participants’ residential stays, community-based housing stabilization following placement, connection to benefits and supportive services, and discharge planning.
The position maintains a caseload of approximately 16-20 households and works directly with participants from referral and intake through permanent housing placement, stabilization, and program exit.
Through supportive, respectful, and trauma-informed relationships, the Family Housing Case Manager I helps participants identify and address barriers to securing and maintaining permanent housing, including those related to income, mental health, trauma, substance use, and other complex needs. The position works both independently and as part of the HFF program team and is responsible for timely documentation, HMIS data entry, file compliance, performance monitoring, and participation in the agency’s emergency on-call rotation.
