LPN I-New Graduate- BMH Med/Surg role at Centra Health
The Licensed Practical Nurse (LPN) practices under the direction or supervision of a licensed medical practitioner, a professional registered nurse, or other licensed health professional authorized by regulations of the Department of Health Regulatory Boards. The LPN coordinates and manages the care provided to assigned patients. Under the supervision of the professional registered nurse (RN), LPNs perform care appropriate to the patient population served and meet daily routine patient care needs.
The LPN performs their duties based on the unit care delivery model. This role collaborates with the interdisciplinary team to deliver care as outlined by the RN. The LPN adheres to a culture reflective of Centra's best practices for quality and service, creates and maintains a safe patient environment, and contributes to a healthy work environment. The practice of the LPN reflects the American Nurses Association's Code of Ethics for Nurses and the Nurse Practice Act of Virginia.
- Under supervision of an RN, plans, coordinates, manages, and documents the care provided to assigned patients as appropriate
- Delegates to appropriate care providers
- Thinks critically as evidenced by appropriate clinical decision-making and prioritization
- Communicates and collaborates with the interdisciplinary team to plan patient care
- Creates and maintains an environment that protects patients from harm
- Adheres to National Patient Safety Goals (NPSG)
- Participates in initiatives resulting from quality indicators and/or unit-based quality metrics
- Reinforces appropriate patient and/or caregiver education with utilization of applicable resources
- Promotes and sustains a culture reflective of Centra's best practices
- Operationalizes change resulting from evidence-based practice or research
- Applies service excellence best practices in approaching patients, visitors, physicians, and colleagues
- Participates in service recovery and uses appropriate resources during complex situations
- Handles conflict resolution in an appropriate, timely, and consistent manner
- Identifies personal contributions to unit-based cascading goals
- Contributes to processes that support efficient patient flow
- Uses resources (e.g., time, people, supplies, etc.) cost effectively
- Engages in own professional development as evidenced by self assessment and attainment of degrees, certifications, and continuing education
- Contributes to providing a supportive environment that is conducive to learning for all students from all academic programs

Education:
Graduate of an accredited Practical Nursing Program - Required Certifications and Licensures:
Hold a current, active license as a practical nurse in Virginia or hold a current multistate licensure privilege as a practical nurse. Hold a current, active American Heart Association Basic Life Support (AHA BLS) course completion card.
