As a Matrix provider, you’ll conduct thorough health assessments that last up to one hour, gaining invaluable insights into your patients’ health. Your responsibilities will include:
- Conducting comprehensive assessments, including medical history, diagnosis, and treatment plans.
- Administering and ordering necessary tests.
- Collaborating with healthcare teams, including physicians and social workers.
- Providing care in patients’ homes or skilled nursing facilities.
- Master’s Degree or equivalent experience with NP or PA licensure.
- Current NP or PA licensure with prescriptive authority.
- Board certification in relevant specialties.
- Current BLS, ACLS, or CPR certification.
- 1 year of experience preferred, but new grads are encouraged to apply!
- Strong communication skills and proficiency in medical record software.
