Social Services Director – Full-Time Visalia Post Acute

Pay Range: $27.00-$28.00 per hour, depending on experience

Make a Meaningful Difference Every Day Visalia Post Acute is seeking a compassionate, organized, and experienced Full-Time Social Services Director to support the emotional, social, and psychosocial well-being of our residents. This position works closely with residents, families, nursing staff, therapy professionals, community agencies, and the interdisciplinary care team. The ideal candidate is a strong advocate who can manage sensitive situations with professionalism, objectivity, persistence, and confidentiality.

A minimum of a bachelor’s degree is required for this position.

Essential Duties and Responsibilities
  • Assess residents' social, emotional, psychological, and family needs.
  • Develop appropriate goals and courses of action in collaboration with residents and their families.
  • Provide medically related social services that help residents achieve or maintain their highest practicable level of physical, mental, and psychosocial well-being.
  • Assist with developing, reviewing, and updating social-services care plans.
  • Help residents achieve the highest possible level of independence, self-care, and overall well-being.
  • Provide emotional support to residents and families coping with illness, disability, loss, grief, or changes in condition.
  • Counsel residents and family members regarding adjustment, care decisions, and available resources.
  • Assist residents and families with healthcare decisions and advance-care planning as appropriate.
  • Participate in interdisciplinary care conferences and communicate resident needs to the care team.
  • Coordinate discharge planning with residents, families, facility departments, community agencies, equipment providers, and outside service organizations.
  • Arrange referrals and connect residents with appropriate community, health, and social-service resources.
  • Assist with scheduling outpatient appointments and on-site ancillary services, including optometry, podiatry, dentistry, and psychiatric services.
  • Assist residents in obtaining appropriate prosthetics, assistive devices, and other necessary resources.
  • Maintain accurate, timely, and confidential social-services documentation.
  • Assist with personal shopping and other resident-support needs as appropriate.
  • Help inventory and track residents' personal belongings.
  • Coordinate responses to reports of missing, lost, or stolen resident property.
  • Develop and maintain strong relationships with facility departments and outside community agencies.
  • Provide staff education and in-service training as requested.
  • Advocate for resident rights, dignity, preferences, and quality of life.
  • Maintain the confidentiality of all resident health and personal information.
  • Perform additional duties as assigned.
Minimum Qualifications
  • Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or a closely related field required.
  • At least two years of supervised social-work experience in a healthcare setting working directly with individuals required.
  • Master of Social Work degree preferred.
  • Skilled nursing, long-term care, rehabilitation, or geriatric experience strongly preferred.
  • Knowledge of resident rights, discharge planning, care planning, community resources, and psychosocial assessment.
  • Experience with Point Click Care  or another electronic health-record system preferred.
  • Strong counseling, crisis-management, advocacy, and problem-solving abilities.
  • Excellent written and verbal communication skills.
  • Ability to manage sensitive information with discretion and confidentiality.
  • Strong organization, documentation, planning, and time-management skills.
