We are currently seeking to fill the Registered Nurse position to serve in Kings County and surrounding areas.

Patient Care
  • Completes an initial, comprehensive and ongoing assessment of the patient and family to determine hospice needs.
  • Provides a complete physical assessment and history of current and previous illnesses.
  • Provides professional nursing care using all elements of the nursing process as defined by the state Nurse Practice Act.
  • Assesses and evaluates patient status and writes and initiates the plan of care.
  • Re‑evaluates patient and family/caregiver needs regularly and revises the plan of care as necessary.
  • Uses health assessment data to determine nursing diagnoses.
  • Develops a care plan that establishes goals, incorporates palliative nursing actions, and includes the patient and family in planning.
  • Initiates appropriate preventive and rehabilitative nursing procedures and administers medications and treatments as prescribed.
  • Counsels the patient and family regarding nursing and related needs.
  • Provides health‑care instructions and assists the patient with activities of daily living to encourage self‑sufficiency and optional comfort care.
  • Acts as case manager when assigned by the clinical supervisor and coordinates patient care for the assigned caseload.
Communication
  • Completes, maintains and submits accurate clinical notes about patient condition and care provided.
  • Communicates with physicians regarding patient needs, reports changes in condition, and obtains orders as required.
  • Coordinates care plans with community health personnel.
  • Teaches patients and family/caregivers self‑care techniques and provides medication, diet and other nursing instructions.
  • Maintains a safe environment for the patient.
  • Assists family/caregivers and other team members in ensuring continuity of care.
  • Works with the interdisciplinary group to meet emotional needs of patient and family.
  • Attends interdisciplinary group meetings.
Additional Duties
  • Participates in on‑call duties as defined by the on‑call policy.
  • Ensures arrangements for equipment and other necessary services are available.
  • Supervises ancillary personnel and delegates responsibilities when required.
  • Assumes responsibility for personal professional growth and maintains, upgrades knowledge and practice skills through continuing education and in‑service classes.
  • Fulfills the obligation to accept or request case assignments.
  • Actively participates in quality assessment and performance improvement teams and activities.
Qualifications
  • Graduate of a National League of Nursing accredited nursing school.
  • Current California RN license and CPR certification.
  • Minimum of one year professional nursing experience within the last three years, or a baccalaureate degree in nursing from an NLU‑accredited program and current RN license; preferred two years of experience, at least one in public health, home care, or hospice nursing.
  • Licensed driver with a motor vehicle insured and in good working order as required by state and/or organization.
  • Self‑directed and able to work with minimal supervision; no management experience required.
  • Excellent observational, verbal, written, problem‑solving, basic math, and nursing skills per competency checklist.
  • Physically capable of prolonged walking or standing, lifting, positioning and transferring patients, and carrying supplies and equipment; able to perform necessary visual and hearing tasks for nursing duties.
