Registered Nurse (RN):
Porterville and Surrounding Areas
Compassionate Care Home Health is looking for a dedicated Registered Nurse to serve patients in Porterville and surrounding areas.

- Hourly and paid per visit (PPV) hybrid model, negotiable based on experience, skill level, availability
- Opportunities to accumulate overtime
- Complete comprehensive assessments of patients and families to determine hospice needs
- Provide professional nursing care by utilizing all elements of the nursing process
- Assess and evaluate patient status, write and initiate plan of care, re-evaluate regularly
- Develop and revise care plans incorporating palliative nursing actions, involve patient and family in planning
- Administer medications and treatments as prescribed, provide counseling, give health care instructions
- Assist with activities of daily living, facilitate self-sufficiency and comfort care
- Act as case manager when assigned, coordinate care for caseload
- Maintain accurate clinical notes, record pain and symptom management outcomes
- Communicate with physicians regarding patient needs and orders, coordinate with community health providers
- Teach self-care techniques to patients and families, support interdisciplinary teamwork
- Ensure safe environment, comply with policies, participate in interdisciplinary meetings
- Participate in on-call duties as defined by policy, ensure equipment availability
- Supervise ancillary personnel, delegate responsibilities, pursue continuing education, participate in quality improvement activities
- Graduate of a National League of Nursing accredited school of nursing
- Current license in California and current CPR certification
- Minimum one year of professional nursing experience within last three years OR baccalaureate degree with current RN license; preferred two years experience in public health, home care, or hospice nursing
- Licensed driver with insured automobile in good working order
- Responsible for supervising hospice aides, but management experience not required
- Self-directed, able to work with minimal supervision
- Excellent observation, communication, problem solving and basic math skills; nursing skills per competency checklist
- Physical ability to lift, transfer patients, provide prolonged care; adequate vision and hearing
