The MDS Coordinator (RN) is responsible for the accurate and timely completion and submission of the Minimum Data Set (MDS) for all residents admitted to the Skilled Nursing unit of Lions Gate. This role ensures compliance with State and Federal requirements, supports resident-centered care planning, and serves as a liaison between the clinical team and regulatory bodies.

• Complete and submit all required MDS assessments, including admission, quarterly, annual, change-of-status, and Medicare assessments, within established time frames.
• Ensure accuracy and completeness of all sections of the MDS, coordinating sign-offs from the interdisciplinary healthcare team.
• Prepare and manage care conference lists for new admissions and quarterly reviews.
• Facilitate weekly Medicare meetings and maintain an updated Medicare schedule with projected dates.
• Collaborate with Nursing, Therapy (PT, OT, ST), Dietary, Activities, and Social Services teams to ensure comprehensive resident care.
• Coordinate monthly care conference schedules with the Social Worker.
• Review and update resident care plans as needed to ensure alignment with MDS findings.
• Participate in morning meetings and contribute to interdisciplinary discussions.
• Complete managed care updates and other related coordination tasks.
• Perform other duties as assigned by the supervisor.

• Current Registered Nurse (RN) license in the State of New Jersey (required).
• RNAC (Resident Assessment Coordinator) Certification – REQUIRED. Candidates without active RNAC Certification will not be considered.
• Minimum 3 years of experience in MDS coordination or related skilled nursing facility roles.
• Thorough knowledge of MDS requirements, Medicare regulations, and RAI processes.
• Proficient in computer applications and electronic medical record systems.
• Strong communication skills—both verbal and written—in English.

• Adheres to company and departmental dress code and appearance standards; wears identification at all times while on duty or representing Lions Gate.
• Demonstrates dependability and punctuality by reporting to work as scheduled.
• Complies with all Lions Gate policies and ethical standards as outlined in the Corporate Compliance Plan.
• Maintains compliance with all State, Federal, and OSHA safety regulations.
• Completes mandatory annual education requirements (Corporate Compliance, HIPAA, etc.).
• Maintains resident confidentiality and upholds residents’ rights.
• Fosters positive and professional relationships with residents, families, and staff.
• Ensures work practices maintain the Kosher standards of the facility in identified areas.
• Participates in departmental meetings and professional development opportunities.
