Hospice Registered Nurse Case Manager (RN)
Starting Salary: $90,000 / Year
Responsibilities- Completing an initial assessment of patient and family to determine hospice care needs
- Regularly re‑evaluating the patient’s nursing needs
- Using assessment data to determine care plan needs
- Collaborating with the entire healthcare team, patient and family to help direct the work toward continuous, well‑coordinated care
- Counseling the patient and family in meeting end‑of‑life needs
- Initiating appropriate preventative and palliative nursing procedures
- Communicating with the physician regarding patient’s needs and reporting any changes in the patient’s condition
- Supervising the hospice aides
Associate’s Degree in Nursing
Professional Experience- State‑Certified RN license
- Two years of professional nursing experience (hospice experience preferred)
- Valid driver’s license and reliable transportation
- Willingness to travel
- Strong verbal and written communication skills
