Government Programs Care Manager III – Texas Counties
Join Health Care Service Corporation as a Government Programs Care Manager III serving the following Texas counties:
Brazos, Limestone, Freestone, Leon, Falls, Robertson, Madison, Milam, Grimes, Burleson, Washington, and Bell.
This position is responsible for providing care coordination and transition planning to members via telephonic and/or field-based care management. The role involves educating members on health care programs, supporting adherence to personal health goals, responding to member inquiries, supporting clinical operations with provider and member activities, and transitioning members to adult programs. Responsibilities also include completing member phone and field health screenings, comprehensive health assessments, and care planning duties, which may require visits to members’ homes or facilities to support complex and specialty populations.
Job Requirements- Registered Nurse (RN) with a current and unrestricted license to practice in Texas and at least 2 years of direct clinical care experience in a clinical setting, OR Licensed Professional Counselor (LPC), Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW) or Licensed Marriage Family Therapist (LMFT) with a Texas current and unrestricted license and at least 2 years of clinical practice experience.
- Current, valid, unrestricted license in the state of operations (or reciprocity). For compact licensee changing permanent residence to the state of operations, you must obtain active, unrestricted RN licensure in the state of operations within 90 days of hire.
- At least 3 years of wellness or managed care experience presenting clinical issues with members/physicians.
- Experience working with level 3 children with behavioral health issues and with children and young adults with similar conditions or behaviors within the last five (5) years.
- Knowledge of the health and wellness marketplace and employer trends.
- Verbal and written communication skills including discussing medical needs with members and interfacing with internal staff/management and external vendors.
- Analytical experience including medical data analysis.
- PC proficiency in Word, Excel, PowerPoint, and experience with database and web-based applications.
- Current unrestricted driver’s license, transportation and applicable insurance.
- Ability and willingness to travel within the assigned territory.
Job Requirements

- Medicaid experience.
- Experience with Texas STAR Kids and/or STAR Plus government programs.
- Community outreach experience.
- Population management experience.
- Guardianship experience.
- Experience working with IDD/Autism population.
- Texas Workforce Commission experience.
- Certification in Case Management.
- Bilingual in English and Spanish.
- Population health management experience.
- Qualified to be a Telecommuter based in one of the specified Texas counties.
- Willingness to meet in the Waco office quarterly as needed.
- Work hours:
Monday through Friday, 8 AM to 5 PM CST.
- This is a field position; meeting members in person in their homes is required.
- Travel within an assigned mile radius is required; mileage paid according to IRS guidelines.

- Compensation: $26.90 – $59.35 per hour, exact amount may vary based on skills, experience, and location.
