Position SummaryThe Home Health Field Licensed Vocational Nurse (LVN) delivers skilled nursing care in the patient’s home under the supervision of a Registered Nurse (RN) and in accordance with the physician’s plan of care.
Key Responsibilities- Patient Care Delivery:
Provides skilled nursing services such as wound care, medication administration, catheter care, and disease process teaching in alignment with the plan of care. - Clinical Observation & Reporting:
Monitors patient status, documents findings accurately, and promptly reports changes in condition to the supervising RN or clinical manager. - Care Plan Implementation:
Follows the established plan of care, ensuring all interventions are completed as ordered and within scope of practice. - Patient & Caregiver
Education:
Teaches patients and caregivers about medications, treatments, and self-care techniques to support independence and recovery. - Collaboration with Care Team:
Coordinates with RNs, therapists, aides, and other healthcare professionals to ensure comprehensive patient care. - Compliance & Documentation:
Completes visit documentation accurately and timely in accordance with Medicare, Medicaid, and agency requirements. - Safety & Infection Control:
Follows proper infection prevention, safety, and emergency protocols to protect patient and staff well-being. - Customer Service:
Builds positive, respectful relationships with patients and families, promoting trust and satisfaction with home health services.
- Excellent organizational and time-management abilities.
- Effective communicator with the ability to work collaboratively across departments.
- Skilled in process improvement and problem-solving in a fast-paced environment.
- Office-based with regular travel to branch locations within the assigned region.
- Fast-paced and deadline-driven environment requiring flexibility and adaptability.
- Prolonged sitting and computer use.
- High level of mental concentration and ability to manage multiple operational priorities.
