DescriptionThe Physical Therapist will provide outpatient PT services in an ethical, professional manner, in accordance with state guidelines. Duties include evaluating patients, establishing treatment plans, supervising PTAs, delivering manual therapy and therapeutic exercises, educating patients and families, documenting care, submitting charges, communicating with physicians, and participating in staff meetings. Additional duties are assigned as needed.
Essential Functions- Evaluate and treat patients in accordance with state guidelines, identifying root causes of dysfunction that directly translate to impairments in daily function.
- Establish appropriate plans of care (POC) for patients, outlining the path to achieve their goals and reassessing periodically.
- Oversee the treatment provided by PTAs based on established POC.
- Deliver treatment including manual therapy, therapeutic exercises, and training on movement and body mechanics.
- Educate patients and families on self‑management and proper techniques.
- Update POC as necessary and assess adaptive equipment needs.
- Develop discharge plans, document treatment, and submit charges.
- Communicate regularly with physicians and medical staff regarding patient status.
- Assist in developing and updating Odom Rehab treatment protocols.
- Actively participate in staff meetings.
- Track and complete continuing education credits and licensure renewal.
- Maintain an active physical therapy license.

- Follow the employee handbook.
- Graduation from an accredited Physical Therapy program with a degree in Physical Therapy and current Minnesota state licensure.
- One year of experience as a Physical Therapist (preferred).
- Experience in a primary healthcare setting (preferred).
- Ability to follow written and oral directions.
- Computer skills.
- High level of personal and professional accountability and responsibility.
- Self‑directed and performs duties independently.
- Problem‑solving and critical thinking skills.
- Maintains confidentiality.
- Valid Minnesota driver’s license, satisfactory driving record, and reliable vehicle (mandatory for administrative driving).
- Maintains personal vehicle auto insurance coverage.
Physical Requirements
Medium work: lifting no more than 50 pounds at a time and carrying objects up to 25 pounds. Includes both sedentary and light work.
Driving RequirementsThis position requires mandatory administrative driving. Employees must maintain a satisfactory driving record and comply with OHW’s driver safety requirements.
