Position: RN Coordinator Stroke 7170

Purpose of Job

The Stroke Program Coordinator provides leadership for the nursing care and management of patients experiencing a stroke. The coordinator works both independently and in collaboration with the Stroke Medical Director and other members of the stroke team to ensure all patients of the institution receive high quality evidence-based care through practice oversight.

Job Functions
  • Participates in the development and planning of goals and objectives related to the care of stroke patients.
  • Coordinates with the Stroke Medical Director and clinicians to assess the need for policies, procedures and protocols based on best practice relating to the care of stroke patients.
  • Develops and implements policies and procedures; makes recommendations for revisions; assists with updating policies and procedures based on current literature.
  • Supervises adherence to hospital policies, procedures and standards through observation, medical record review, staff feedback and other appropriate resources.
  • Implements program initiatives.
  • Coordinates preparation for the statewide stroke designation and re-designation site visits.
  • Develops and fosters collaborative relationships with all hospital departments to facilitate and support quality stroke care.
  • Serves as a resource for the hospital staff regarding stroke care issues.
  • Plans and implements strategies for ongoing Stroke Program development and improvement.
  • Collaborates with the Stroke Medical Director, physicians, and other health care professionals to provide clinical and system oversight for the care of stroke patients, ensuring the provision of efficient, quality care.
  • Assists the Stroke Medical Director in the development, implementation, and evaluation of a quality plan which is multi-disciplinary, and patient‑outcome focused.
  • Monitors stroke patient outcomes; evaluates for trends.
  • Coordinates with physicians, nurses, and other in‑hospital staff to evaluate and address specific patient care issues.
  • Participates in case reviews.
  • Serves as the coordinator for the identification, investigation, reporting and follow up of incidents and quality issues throughout the program while maintaining confidentiality.
  • Monitors the stroke team's compliance with policies and standards.
  • Develops and monitors the Stroke PI Program in collaboration with the Stroke Medical Director.
  • Assists in data collection related to the stroke patient.
  • Analyze registry data on the stroke patient population; identifies trends for strategic planning and performance improvement.
  • Ensures accurate data entry into the stroke registry.
  • Manages registry data; collects and analyzes trends.
  • Maintains stroke log data by keeping information up to date.
  • Plans, coordinates and evaluates stroke related educational programs for nursing staff.
  • Monitors physician and nurse compliance with the educational requirements of the Stroke Program.
  • Seeks and pursues opportunities for internal and external stroke related educational programs for hospital staff.
  • Minimum Education/Work Experience
    • Graduate of an accredited school of nursing
    • Minimum of three clinical experience in stroke/emergency care
    Preferred Qualifications
    • Stroke Certified Registered Nurse
    • NIHSS training
    Knowledge/Skills/Abilities
    • Ability to meet and maintain the necessary background checks as aligned with positions functions
    • Ability to communicate in the English language for effective written and verbal correspondence to complete job functions as mentioned above
    • Abilities to establish and maintain effective interpersonal relationships
    • Ability to design and implement change
    • Ability to problem solve‑make decisions
    • Critical thinking, analytical, research and teaching/coaching skills
    • Familiar with comprehensive stroke program requirements and measures
    License/Certifications
    • Current registered nurse licensure in Minnesota or if licensed in another state, a permit from the Minnesota Board of Nursing while the license application is in process
    • BLS Certification
    • ACLS Certification
