Position Summary:We are hiring a field-based Nurse Practitioner (AGNP or CNP) who thrives in an independent, flexible environment. If you’re not yet certified in wound care, we’ll pay for it. This is a great fit for someone looking to grow their clinical specialty while making a direct impact on a vulnerable population.
Key Responsibilities:- Visit assisted living, memory care, and senior communities to provide primary care
- Address wounds, chronic conditions, and preventative care in place
- Partner with care teams and families to ensure holistic support
- Keep accurate records and communicate clearly with all stakeholders
- AGNP certification preferred
- Experience with geriatrics, family practice, or long-term care
- Must have a valid driver’s license and auto insurance
- Strong documentation, collaboration, and communication skills
