Job Summary - Night Shift. The Registered Nurse (RN) provides direct patient care in a skilled nursing home environment, ensuring high-quality clinical services that meet federal, state, and facility standards. The RN supports residents with complex medical needs, coordinates care with interdisciplinary teams, and promotes a safe, compassionate, and supportive environment.

The RN supports residents with complex medical needs, coordinates care with interdisciplinary teams, and promotes a safe, compassionate, and supportive environment.
Responsibilities- Provide and coordinate direct nursing care to residents in accordance with care plans and regulatory requirements.
- Perform assessments, monitor vital signs, and evaluate changes in resident health status.
- Administer medications, IV therapy, treatments, and wound care according to physician orders.
- Maintain accurate, timely documentation in electronic health records (EHR).
- Ensure proper infection control practices and adherence to safety protocols.
- Communicate effectively with physicians, nurse practitioners, therapists, CNAs, and other team members.
- Participate in resident care conferences and contribute to the updating of individualized care plans.
- Support transitions of care, including hospital transfers and post-acute admissions.
- Supervise Certified Nursing Assistants (CNAs).
- Provide guidance, training, and performance feedback to assistive staff.
- Ensure staffing assignments meet resident acuity and facility standards.
- Educate residents and family members on care procedures, medications, and treatment plans.
- Respond to resident concerns with professionalism, empathy, and respect.
- Advocate for resident rights and overall well-being.
- Adhere to state and federal regulations (e.g., CMS, OBRA, state board of nursing).
- Participate in quality improvement initiatives and audits.
- Maintain accurate reports on incidents, falls, skin issues, and changes in condition.
