Position Description
:
The Registered Nurse II Children First, Nurse‑Family Partnership is responsible for providing direct or indirect professional nursing services in a variety of settings. Children First, Oklahoma’s Nurse‑Family Partnership is offered free of charge to first‑time, low resource mothers. A specially trained nurse provides personal visits in the family’s home or other location beginning early in pregnancy and continuing through the child’s second birthday.
Nurses complete nursing assessments utilizing the Nursing Process and provide education related to pregnancy, health, community resources, parenting, safety, child development and other topics as needed. They also support the mom as she works toward attaining life goals that will ensure a more secure future for the family.
Position Responsibilities /
Essential Functions:
- Prepares for and actively participates in reflective supervision, case‑conferences and team meetings.
- Monitors and assesses the health status of first‑time mothers and infants to age two utilizing the Nurse‑Family Partnership Guidelines, program guidelines and Physician Approved Protocols.
- Works with the client/family to develop long‑term and short‑term personal and family goals.
- Develops a plan of care based on nursing assessment and provides nurse case management and health education.
- Delivers professional nursing care in the client’s home or other setting.
- Completes electronic documentation in a timely frame and format as outlined by the Children First Program.
- Assures quality health care through use of various measures such as record review, peer review, direct observation, assessment of individual, family, and community for the desired outcome.
- Assists with activities in support of public health emergency response.
- Works with families following a comprehensive program plan that focuses on the client’s development of personal health, parenting, environmental health, preventing unintended subsequent pregnancies, and finding or returning to work and school.
- Completes required training and ongoing education as outlined by the NFP Model and Children First Program Guidelines.
- Achieves and maintains caseload as outlined by the Children First Program Guidelines.
- Collaborates with internal and external partners.
- Being present in the office is an essential function of this job.
- Other duties as assigned.
