Job SummaryProvides case management services to Kaiser Permanente Quest Integration (QI) members and their families to help navigate personal, social, financial, and medical needs. Works with QI RN Manager to assign QI members with special health care needs to appropriate care managers or care coordinators.
Essential Responsibilities- Develop the QI care management toolbox and related checklists for initial and reassessment home visits.
- Receive and perform initial triage of paraprofessionals; assist with resource development and network with community‑based services.
- Facilitate referrals to Home Community Based Services, Durable Medical Equipment, dental services, social/behavioral supports, child welfare services, adult protection services, and developmental services.
- Provide member care services to QI enrollees, navigating medical, social, and psychosocial needs, and educating members on the QI program.
- Navigate transportation and housing needs for members and families for medical services both inter‑island and on the mainland.
- Collaborate with the RN manager to assign QI caseloads and develop HFA tracking forms for home visits and follow‑ups.
- Interpret and explain QI program services; act as member advocacy and liaison to promote Kaiser philosophy and confirm QI coverage to patients and providers.
- Three (3) years community outreach in a healthcare setting, or related experience.
- High school diploma or GED.
- Driver's License (from any state) required at hire.
- Demonstrated knowledge and skill in adaptability, customer service, interpersonal relations, oral communication, problem solving, teamwork, and written communication.
- Demonstrated knowledge and skill in word processing, spreadsheet and database PC applications.
- Trauma situation and CTN/Special Processes experience.
- Bachelor's degree in health and human services, or related field.
