Anti-Coagulation Practitioner

Overview

The primary role of the post will be to work in the Pathology-based Anticoagulation service, a sub discipline of the Pathology Patient Services Department.

The Anticoagulation Practitioner will competently use several clinical systems including Dawn AC, Systm One and the Pathology Laboratory Information System (LIMS) to ensure the welfare of all patients under the care of the Pathology Patient Services department.

The Anticoagulation Practitioner will be responsible for the monitoring of the efficacy of VKA anticoagulation therapy and provide clinical guidance to patients in relation to lifestyle choices which may affect this.

This post is for four days/week (day off Wednesday). The working hours are 08:45 - 17:00 with a requirement to work a late shift on Friday every 5 weeks finishing  role is primarily office based and patients are contacted by telephone. There is also a requirement to attend patient facing clinics at the Community Diagnostic Centre, approximately one shift per week.

To be responsible for continual monitoring and effectiveness of each patient's response to anticoagulant medication and ensuring that records are maintained to the highest possible standard.

Ensure post-discharge patient aftercare is rigidly maintained, liaising with clinical services, contacting General Practitioners, and accurately recording all correspondence accordingly.

Coordinate the Phlebotomy domiciliary service, liaising with Clinical Teams and General Practitioners and updating clinical systems and recording activity as appropriate.

The Anticoagulant Practitioner liaises with all other specialist fields in the preparation of pre-op and post-op management following policies and procedures.
