Wakefield Council is seeking a Lead Practitioner for Early Help to work within a multi-disciplinary team and support families using a whole-family, strengths-based approach.
The post holder will manage packages of support for families with multiple needs, undertake assessments, develop plans of support, and coordinate multi-agency reviews of the plans. To report on outcomes as part of the Children and Young People’s Performance Framework.
