Tieve Tara Medical Centre which is part of Spectrum Community Health CIC, are looking for a Practice Nurse to join our friendly nursing team. We are a busy surgery with approximately 5,500 patients so an adaptable approach is needed.
We are seeking to appoint an individual with a caring nature who will provide and maintain a professional manner and a high standard of nursing care for our patients.
This is a fantastic opportunity for a Practice Nurse with good communication skills, and a commitment to working as part of a strong and effective team.
Main duties of the jobIdeally the successful applicant will be qualified to undertake chronic disease management (Diabetes, and/or COPD and Asthma), travel health, women's health and all general treatment room tasks. We would be supportive of any training requirements.
Applicants must be a fully qualified Registered General Nurse (RGN) and have a valid NMC number.
- General Treatment Room duties
- Smear Taker Wound Knowledge (including Doppler and Compression)
- Childhood Immunisations
- Travel Immunisations
The flexibility to be able to cover sick/annual leave at times if required.

Management of Chronic Disease- Diagnosis, monitoring and development of individual management plans: agreeing these as appropriate with the patient and other health professionals.
- Checking compliance with the adherence to appropriate treatments using a holistic patient-centred approach.
- Identifying the impact of treatment and implementing or recommending changes as appropriate.
- Provide patients with comprehensive information about potential health risks and chronic disease, using education, discussion and appropriate literature, to promote understanding of lifestyle issues, in particular smoking cessation, healthy eating and regular exercise.
- Working with patients to develop a management plan where health problems or potential health problems are identified
- Provide in depth monitoring and advice as appropriate and work with the patient in deciding on management plans.
- Undertaking a pre-travel risk assessment using the information the patient has provided on their travel form prior to their appointment.
